Registration Documents

  • Currently Enrolled Students:

    • Proof of Residence: Utility bill (Electric, Gas, or Water) must be under the parent's/guardian's name. If under someone else's name, please provide a notarized proof of residence letter from the person they are living with. 
    • Immunization record: Provide proof that immunizations are up to date.

    New Students To The District:

    • Birth Certificate: Government-issued Birth Certificate or other documents suitable as proof of the child's identity and age.
    • Immunization record: Provide proof that immunizations are up to date.
    • Social Security Card: If a child does not have a Social Security number a state number will be assigned.
    • Proof of Residence: Utility bill (Electric, Gas, or Water) must be under the parent's/guardian's name. If under someone else's name, please provide a notarized proof of residence letter from the person they are living with.
    • Child’s withdrawal form/last report card from prior school ( if applicable)
    • Proof of income – Pre K only

    Note: Any new or current out of district student who wishes to enroll for the current or upcoming school year must submit a transfer request application yearly and be approved prior to enrolling in the district.
